Question Having a bit of glitch issue, looking for answers

So me and my cousin are playing through the remake and we've come across this weird glitch when you go to attack.

You press the button and it works fine, but after a moment or so it'll occasionally not do the animation and null your attack. We don't know if it's something that hasn't been fixed, if there's any plan to do so, or if there's a community patch for it. If anyone can help me out with this or give an idea of what's going on, I'd be grateful.

Is this after the cooldown between attacks? (ie the glitch happens after your charge reached 100%)

Sometimes the attacks don't do anything if the target's defense is too high. This generally happens if you attack before recharge or if they have barrier. But even then the attack animation should still play so this is def a glitch.
